Setting/Population: Neuro Outpatient Rehab for Adults with a Stroke or Brain Injury
Job Summary:
We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join the Neuro Clinic of Idaho for Stroke & Brain Injury team in Meridian. The ideal candidate will have experience working with neurological conditions and a passion for helping patients regain communication, cognitive, and swallowing functions. This role involves assessing, diagnosing, and treating adults with speech, language, voice, cognitive, and swallowing disorders related to neurological impairments such as stroke, traumatic brain injury (TBI), tumor resection, and other brain injuries, as well as neurological conditions such as Parkinson’s disease, multiple sclerosis (MS), and amyotrophic lateral sclerosis (ALS).
Key Responsibilities:

Conduct comprehensive evaluations for speech, language, cognitive-communication, and swallowing disorders.

Develop and implement individualized treatment plans based on patient needs.

Provide evidence-based therapy for neurological conditions affecting speech and swallowing.

Collaborate with an interdisciplinary team, including neurologists, occupational therapists, physical therapists, and other healthcare professionals.

Educate patients and caregivers on treatment strategies, home exercises, and compensatory techniques.
-
Utilize advanced therapy techniques and technologies, such as neuromuscular electrical stimulation (NMES), voice therapy, and augmentative and alternative communication (AAC) devices as patients need.

Maintain accurate documentation in compliance with clinic and regulatory standards.

Participate in clinic meetings, case conferences, and continuing education opportunities.
Qualifications:
-
Master’s degree in Speech-Language Pathology from an accredited program.
-
Current state licensure as a Speech-Language Pathologist in Idaho.
-
Certificate of Clinical Competence (CCC-SLP) from ASHA preferred.
-
Experience working with Adults diagnosed with neurological disorders in an outpatient, rehabilitation or acute care setting preferred.

Strong knowledge of neurogenic communication disorders, dysphagia management, and cognitive-linguistic therapy.

Preference to Insurance credentialed therapists.
-
Proficiency in instrumental assessments (e.g., MBSS, FEES) is a plus.

Excellent communication, teamwork, and patient-centered care skills.

Care Transition Coordinator
Position Overview:
The Care Transition Coordinator is responsible for facilitating smooth transitions for patients across various care settings. This role focuses on ensuring that patients receive the necessary support, education, and resources to successfully navigate their healthcare journey.
Key Responsibilities:
1. Patient Interaction:

Greet and schedule patients in a friendly and professional manner.

Answer phone calls appropriately, take messages, and manage scheduling.

Monitor patient flow and assist with daily check-offs and patient numbers.
2. Chart Management:

Create and maintain accurate patient charts, ensuring comprehensive and up-to-date information.

Manage patient insurance and payment information diligently.
3. Financial Management:

Process weekly deposits and maintain accurate records in checkbook and accounting software (e.g., Quicken).

Track and record financial transactions, ensuring all entries are accurate.

Balance the checkbook monthly and maintain accounts payable, ensuring timely bill payments.

Research and resolve billing discrepancies or inquiries as necessary.
4. Administrative Duties:

Open, distribute, and respond to daily mail.

Fax patient documents to physicians as needed and ensure all paperwork is completed accurately.

Complete and mail patient correspondence to appropriate parties.

Verify insurance information for new patients and facilitate recertifications for claims.
5. Supply Management:

Order and maintain inventory of necessary forms, envelopes, business cards, and office supplies.

Purchase items essential for operations and maintain a supply inventory.
6. Reporting:
-
Compile and complete designated weekly and monthly reports, including office goals and patient counts.

Notify therapists of any pertinent patient information and updates.
7. Team Support:
-
Assist with all duties of an aide as needed to ensure smooth office operations.

Coordinate with healthcare providers to streamline patient transitions and enhance care continuity.

Maintain and update lists of vocational counselors, doctors, and other key contacts for marketing purposes.
8. Evening Duties:
-
Contact patients scheduled for the following day to provide appointment reminders.

Prepare credit card reports, organizing receipts and documentation for corporate submission.
Qualifications:
-
High school diploma or equivalent; a degree in healthcare administration or related field preferred.

Previous experience in a healthcare or administrative role is beneficial.

Strong communication and interpersonal skills.

Proficiency in using office software (e.g., GoogleDocx, GoogleSheets, Microsoft Office, Quicken).

Ability to multitask and manage time effectively.
